Reasonable Accommodation Process for COVID-19 Related Requests

For students

Students who wish to request academic assistance for the following circumstances are encouraged to contact their Dean of Students office:

  • Have tested positive for COVID-19. The Wellness Center will provide medical verification for students who are under their care;
  • Are required to quarantine from exposure to or symptoms of COVID-19;
  • Are at higher risk due to age or pregnancy; or
  • Live with someone who is at higher risk due to underlying health conditions, pregnancy, or age, or who has contracted COVID-19.

Students who need assistance for the following circumstance are encouraged to contact Student Disability Services:

  • Have underlying chronic health conditions and are at greater risk for contracting COVID-19;
  • Are experiencing on-going symptoms (beyond three weeks of infection) and secondary medical issues as a result of contracting COVID-19.

To initiate a request for a COVID-related accommodation, students should go to the Student Disability Services (SDS) website and click on the “Registering with SDS” button. The link will lead to the Request for Accommodations Form the completion of which will initiate a request for reasonable accommodations. Treating health care professionals should complete the Medical Verification Form, available on the SDS website.

Students who are currently registered with SDS should contact the SDS office to discuss the academic assistance needed or submit a request for additional accommodations form via the AIM student portal.  SDS will work with students and treating health care professionals to provide appropriate and reasonable accommodations in an expedited manner.

For faculty, other academic appointees, and postdoctoral researchers

Faculty, other academic appointees, and postdoctoral researchers will need to complete the COVID-19 Reasonable Accommodation Request Form and submit the Medical Provider Form, unless the request for accommodation is based solely on age or reasons unrelated to their own underlying health conditions in which case the Medical Provider Form is not required.  Equal Opportunity Programs (EOP) staff will review and coordinate the interactive process for submissions made by faculty, other academic appointees, and postdoctoral researchers. After the request form and Medical Provider Form (where appropriate) have been submitted, a representative from EOP will initiate contact to discuss the request. The EOP representative also will discuss the requested accommodation with the appropriate academic or administrative leader to make a determination, however information about the requester’s specific medical condition will not be shared with supervisors or other departmental representatives.

For staff employees

Staff employees need to complete the COVID-19 Reasonable Accommodation Request Form and submit the Certification of Healthcare Provider Form and the Authorization for Release of Information. Those who submit an accommodation based solely on age or reasons unrelated to their own underlying health conditions are not required to fill out the Certification of Healthcare Provider Form/Authorization for Release of Information. The Employee and Labor Relations team will review requests and coordinate the interactive process for submissions made by staff.  After the request form and Certification of Healthcare Provider Form and Authorization for Release form (where appropriate) have been submitted, a representative from ELR will initiate additional communications related to needs. The ELR representative also will discuss the requested accommodation with the appropriate academic or administrative leader to make a determination. Information about specific medical conditions will not be shared with supervisors or with other departmental representatives.

Students are encouraged to contact Student Disability Services with questions regarding accommodations for students with disabilities, students at high risk according to the CDC, or students with persisting conditions after contracting COVID-19.

Students with questions regarding other academic-related adjustments are encouraged to contact their Area Dean of Students.
